004142 The Heart Transplant Coordinator (HTC) is responsible for the coordination of required testing, management and follow up across the continuum from evaluation, pre-transplant (waitlist management), during transplant and post-transplant. The HTC will maintain regulatory integrity and adherence to established regulations throughout all processes. Educate patients, families, and other care givers on the transplant process through the continuum. The HTC will have on-call responsibilities for the heart transplant program, which includes collaborating with Gift of Life, medical and surgical teams on the inpatient and outpatient services, all members of the multidisciplinary team and consulting specialists as indicated. The transplant coordinator will also support the outpatient transplant clinics.
