Overview
The Structural Heart Coordinator is responsible for providing a deliberate patient and family partnership with the structural heart care team that facilitates a seamless patient experience between the Cardiology Clinic, CT Surgery Clinic, and hospital settings. The Structural Heart Coordinator works to maximize the patient's health outcome and the patient experience by identifying patient and family needs, completing pre and post visit planning, coordinating diagnostic testing and consultations leading up to structural heart procedures. As a member of the interdisciplinary team, contributes professional nursing knowledge and skills in the provision and management of care to patients and their families. Responsibilities
- Coordinates care across the continuum for the valve clinic/structural heart patient populations, serving as a single point of contact by facilitating and overseeing transitions in care within the hospital and Cardiology and CT Surgery Clinics
- Reviews initial patient referrals to the Valve Clinic to ensure appropriate testing per Valve Clinic protocols, national practice guidelines and quality standards
- Coordinates all required consultations and ensures all required provider documentation is completed according to NCDR, Medicare and UnityPoint requirements
- Works with structural heart disease physicians / ARNP, makes adjustments to schedule as necessary throughout the day and follows up with diagnostic testing and procedures as they progress.
- Assesses patient and family learning readiness and provides education related to all diagnostic testing and structural heart procedures.
- Coordinates the scheduling of hospital procedures and pre-surgical visits
- Assists hospital with chart preparation ensuring all required documentation is present
- Actively participates in direct patient care (APP)
- Works with the structural heart physicians / ARNP to coordinate patients' care immediately post-procedure, to ensure appropriate follow up care in the hospital by the structural heart team
- Participates in Valve Clinic within UPH Cardiology Clinic for the care of patients in follow-up
- Triages all scheduled and unscheduled patient visits and phone calls
- Facilitates the multidisciplinary valve conference and updates as patient moves through the evaluation process
- Works with the Outcomes department to facilitate the completion of the TVT Registry Coordinates review of registry metrics and outcomes with the structural heart team 10%
Qualifications
- Completion of an accredited Bachelor of Science nursing program and/or be eligible to take licensure exam. Completion of an accredited advanced practice nursing program; master's degree in nursing.
- Minimum 3 years of cardiovascular (office or hospital) nursing experience.
- Possess a current Iowa RN license. Possess a current Iowa ARNP license; current state and federal DEA certification.
- Maintain current BCLS certification.
- Maintain current ACLS certification
