Maine state licensureto practice as aLicensed Clinical Social Worker (LCSW), Licensed Masters Social Worker - Clinical Conditional (LMSW-CC), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Professional Counselor - Conditional (LCPC-c), Licensed Marriage & Family Therapist (LMFT) or Licensed Marriage & Family Therapist - Conditional (LMFT-C) is required

A Behavioral Health Care Manager (BHCM) is a generalist master's level behavioral health provider whose primary role is focused on building and delivering the Collaborative Care Model (CoCM) within a medical setting.This a full time position, with the option of part-time hours for a candidate looking for a lighter schedule located at our Westbrook MaineHealth Primary Care Internal and Family Medicine, a well-established practice that offers complete care to adults and children.

The CoCM is an integrated behavioral health model of care aimed at achieving improved health outcomes for a population of patients while simultaneously targeting access. The behavioral health care manager (BHCM) is a core member of the Collaborative Care Model working in collaboration with the medical team, consulting psychiatrist, and behavioral health clinicians in the practice. Essential functions of the role include providing behavioral health treatment, supporting and coordinating the mental and physical health care of patients on an assigned caseload with the care team, consulting to and with the care team, linking and referring to additional services as needed, and engaging in additional activities as outlined by the model.

Supervision, connection to a larger behavioral health integration team, quarterly trainings, mentor and peer connections are additional benefits of this position.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Master's Degree in Social Work/Clinical Counseling from an institution accredited by the Council on Social Work/Counselor Education required.

2. License/Certifications: Current applicable state(s) license as an LCSW (Licensed Clinical Social Worker) preferred, or Licensed Clinical Professional Counselor by exception. Valid Driver's license required.

3. Experience: Two years of relevant experience in a health or mental health setting preferred.
