Overview Hospice Continuous/Crisis Care LPN for PRN **Hours: vary 5pm - 8am. Note: Miami Dade County Territory (must live in North or South Dade to be considered for this role). Amedisys is searching for an LPN to join our team of incredible healthcare professionals for our Hospice Care Center located in Hialeah (Dade County). To be able to take care of someone that you don't know when you walk through that door and to embrace them like your own family is great." - ERIN, PATIENT'S DAUGHTER Orientation, Onboarding & Training You will complete your on-boarding working directly with nurses in our program. On average our orientation for an LPN is 3-4 weeks, however, is individualized according to your needs and experience. Responsibilities Verify plan of care with the RN prior to each visit. Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes. Teach patients and caregivers self-care techniques. Check vital signs. Review all medications and perform a medication reconciliation at each visit. Perform specialized procedures ordered by physician (i.e. foley catheter change). Prepare equipment and materials for treatments using the aseptic technique. Complete documentation timely and accurately. Qualifications 1+ year(s) of clinical LPN experience. Current LPN license, specific to the state(s) you are assigned to work. Current CPR certification. HCBH and/or hospice experience a HUGE plus! Valid driver's license, reliable transportation, and liability insurance.
