The Clinical Specialty Coordinator is a Registered Nurse (RN) who coordinates and oversees a program specific in a defined specialty population. The Clinical Specialty Coordinator works across the continuum to develop, implement, and support quality improvement efforts that improve outcomes for the specialty practice. Areas of focus include program development, standardization of practice and documentation, communication, sharing of information, hand-offs, transitions, advocacy, patient and family guidance, education, resources, and specialty specific expertise. The Clinical Specialty Coordinator provides guidance and support to staff in the management, provision and coordination of care in their area of expertise. The certified diabetes care and education specialist (CDCES) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardiometabolic care. The CDCES is an integral part of the interprofessional team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life. The CDCES supports and advocates for people affected by diabetes to optimize quality care. The CDCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. Additional Information

Salary Information Annual Salary Range (Based on 40 hours worked per week): $78,659.24 to $117,988.86Hourly Salary Range: $37.82 to $56.73 Benefits Information Here, you matter.
