Responsibilities

PEER SUPPORT SPECIALIST- BRAND NEW OTP CLINIC

The Peer Recovery Specialists will work with other providers to ensure implementation of the Individualized service plan (with active patient involvement) through appropriate linkages, referrals, coordination, follow-up to needed support services, and direct care services. Specific activities include: assisting patient in their community, follow-up care to assess adherence to medical recommendations, assisting patients with travel in reaching important meetings and appointment as indicated in the treatment plan, and assisting patients with managing their multiple health issues. Through an integrated approach, the Peer Support Specialist utilizes their own experiences to offer individuals assistance in achieving stability and obtaining and maintaining the level of coping and basic life skills that is consistent with their treatment goals. Peer Recovery Support Services are an evidence-based model of care which consists of a qualified peer support provider who assists individuals with their recovery. Peer Recovery Specialists may deliver services in-person or through telemedicine or audio-only.

Responsibilities:

  • Provide service activities to include the following interventions:
  • Education and training of clients and others who have a legitimate role in addressing the needs identified in the Individualized Service Plan
  • Preventive, and therapeutic interventions designed for direct individual activities
  • Assist with skill enhancement or acquisition and support ongoing treatment and functional gains.
  • Develop interpersonal and community relational skills, including adaptation to home, school, work and other natural environments.
  • Therapeutic mentoring and Support
  • Symptom monitoring and self-management of symptoms
  • Inform the client about benefits, community resources and services.
  • Assist the client in accessing benefits and services.
  • Arrange for the client to receive benefits and services.
  • arrange, link or integrate multiple services as well as assessment and reassessment of the client's need for services.
  • Provide coordination of movement across levels of care, directly to the person/family, Referral linkage and Person-Centered Planning
  • Coordinates community re-entry following hospitalization, residential services and other levels of care.
  • Service coordination activities within the person-centered plan
  • Daily and Community Living Skills including Skill building, Socialization skills, Adaptation skills, Symptom monitoring and management skills.
  • Education substance abuse
  • Other duties as assigned.

Qualifications

  • High school diploma or GED required
  • Completion of Certified Peer Recovery Specialist Program
  • CPRS certification required
  • Experience working with individuals with substance use disorder
  • Experience with family, individual and group settings, care coordination, crisis intervention and treatment skills
