Job Overview:

RWJBarnabas Health is seeking a highly dedicated Registered Nurse who has expertise in diabetes.

Responsibilities include:

Accountability for the development and application of Robert Wood Johnson University Hospital Standards of Care, researches evidence based practice related to diabetes self-management to enhance the quality of care too patients with diabetes and their families, provides education and facilitates diabetes self-management in the diabetes self-management education program for outpatients. Educator assesses data relevant to patients health status and interprets the appropriate information needed to identify each patients requirements relative to his or her age-specific and educational needs.

EDUCATION

  • RN, BSN or BS preferred, CDE or CDE eligible

EXPERIENCE

  • Diabetes education experience preferred
  • Proficient in Microsoft Word, Excel, Power Point
  • Must have good computer skills
  • Must have good listening and communication skills
  • Occasionally works under stressful situations at times of high demand or approaching deadlines

Certifications and Licenses Required:

  • BLS and ACLS Certification Required
  • Active New Jersey Registered Nurse License

  • Role is Per Diem Days

Essential Functions:

  • Participates in the development, implementation and evaluation of standards of nursing practice
  • Serves as clinical resource in diabetes for the nursing staff and all members of the health care team
  • Collaborates with the nursing staff and all members of the health care team in providing optimal patient outcomes
  • Demonstrates clinical expertise in diabetes through direct patient care activities
  • Participates in the selection of products related to the care of patients with diabetes, i.e, blood glucose meters
  • Seeks to improve patient and family outcomes through the application of educational concepts and skills
  • Develops and provides education programs for the community
  • Contributes to the educational process and professional development of nursing and dietetic students
  • Contributes to the educational process and professional development of the interdisciplinary team providing care to the patient with diabetes
  • Review, determine and or develop patient education materials appropriate for educational and motivational level of patients with diabetes
  • Maintains current knowledge and skills in diabetes by attending conferences and participating in continuing education activities to increase knowledge in diabetes
  • Maintains BLS, Completes all required Employee Health requirements Completes all required annual RWJ education Maintains current nursing licensure as RN Attains/maintains AADE certification in specialty area as Certified Diabetes
  • Educator Cross trains in nutritional assessment development in individual meal plan and teaching meal planning Consultant Utilizes clinical expertise to provide consultation to inpatients and outpatients
  • Serves as a resource in diabetes to the community including health fairs, glucose screenings, presentations and related events
  • Assists with the development of unit-based performance improvement projects, Reviews research relevant to the specialty of diabetes, Transfers research finding to clinical practice, Identifies opportunities for clinical inquiry, Reviews policies and procedures, updates based on clinical evidence in literature
  • Orients new team members to provide quality education to patients and families, Participates in the implementation and evaluation of institutional goals, Participates in the development, implementation and evaluation of goals for diabetes education, Participates in gathering performance improvement data and evaluation to improve patient outcomes
