Manages the daily operation of the Case Management Department staff to provide effective and efficient discharge planning services to patients in accordance with policies, procedures, regulatory requirements consistent with the mission, vision and values of the Jefferson Health Enterprise.

Essential Functions

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Supervises the Department staff at identified hospitals.
  • Responsible for the recruitment, hiring and onboarding of all new staff.
  • Responsible for updating Departmental policies and procedures, including an annual review of all departmental policies and procedures.
  • In conjunction with the Director, collaborates with Hospital Leadership team to establish and implement departmental functions and goals in accordance with both departmental, hospital and enterprise strategic initiatives and goals.
  • Actively participates in relevant hospital administrative meetings representing the Department.
  • Responsible for completing/participating in completion of annual employee performance evaluations of the staff.

Education:

BSN required. MSN or MBA preferred.

AND

Experience:

Minimum of 5 yearsrsquo; experience in hospital based Case Management experience required. Minimum of 2 years prior management experience required.

Minimum Certifications, Registration or License Requirements:

CCM preferred; active relevant Registered Nurse license in Pennsylvania/New Jersey.
