Summary This position is responsible for collecting and evaluating information about patients; establishing treatment goals with client; determining treatment methodologies; suggesting and exploring resolutions; discussing progress toward goals; evaluating and adjusting treatment provided; supervising student practice and caseload management. Essential Functions Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy. Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education. Plan discharge from care facility to home or other care facility. Plan and conduct programs to combat social problems, prevent substance abuse, or improve community health and counseling services. Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan. Yes Under the direction of the Director of Hospice & Palliative Care and within established organizational and departmental policies and procedures, responsible for providing social casework to clients and families dealing with terminal illness, having difficulty in social/emotional functioning, primarily as it adversely affects the stability of the client's medical condition or the client's response to a prescribed medical regimen. Refer and aid in the referral process to social agencies. Assist the Interdisciplinary Team (IDT) in understanding the significant social and emotional factors to the client's health problem. Bachelor's Degree Social Work required or Master's Degree Social Work required or Master's Degree Mental Health & Behavioral Medicine required Can this role accept experience in lieu of a degree? No Licenses and Credentials Class D Passenger Vehicle Driver's License [State License] preferred Social Worker, Licensed Clinical [LCSW - State License] preferred Social Worker, Licensed Independent Clinical [LICSW - State License] preferred Social Worker, Licensed [LSW - State License] preferred Licensed Mental Health Counselor [State License] preferred Experience Clinical experience in medical setting 0-1 year required Knowledge, Skills & Abilities Excellent organizational and time management skills. Excellent crisis intervention skills. Good problem solving and conflict resolution skills. Ability to work well collaboratively and independently. Strong written and verbal communication skills.
