Diabetes Nurse Program Manager, Endocrinology - Peabody/Salem

We are seeking a 32-hour Diabetes Nurse Program Manager to support our Endocrinology departments. The Program Manager will support our Salem practice located at 331 Highland Ave in Salem, MA on Mondays and Wednesdays from 8-5pm. They will support our Peabody practice located at 2 Corporation Way in Peabody, MA on Tuesdays and Fridays from 8-5pm.
Responsibilities include:
-Developing and implementing comprehensive diabetes management programs, including patient education and support services.
-Providing individualized patient assessments and education on diabetes management, including medication, diet, and lifestyle changes.
-Collaborating with multidisciplinary teams to integrate diabetes care into broader treatment plans and ensure continuity of care.
-Monitoring patient progress and adjusting care plans as necessary to optimize health outcomes.
-Leading training sessions and in-services for healthcare staff on diabetes management protocols and best practices.
-Coordinating and overseeing community outreach and patient engagement activities to promote diabetes awareness and prevention.
-Tracking and reporting on program outcomes, effectiveness, and areas for improvement to inform future initiatives.
-Maintaining up-to-date knowledge of diabetes treatment advancements and integrates new practices into patient care.

Required:

  • Associate's degree in nursing
  • Registered Nurse [RN - State License]
  • 5-7 years of clinical nursing experience
  • 2-3 years of diabetes care or patient education
  • 2-3 years of experience in a supervisory or leadership role


Preferred:

  • Bachelor's degree in nursing
  • Strong clinical knowledge of diabetes management, including medication, nutrition, and lifestyle factors.
  • Excellent communication and teaching skills for effective patient and family education.
  • Ability to collaborate with multidisciplinary teams and integrate diabetes care into comprehensive treatment plans.
  • Proficiency in data collection, tracking, and reporting to evaluate program impact and patient outcomes.
  • Compassionate approach to patient care with a focus on empowering patients to manage their condition.
