DESCRIPTION OF WORK

As an Aging Care Aide 2, you will utilize our computer systems to determine the status of consumer referrals. During this process you will be compiling and recording new referrals primarily received by telephone however, the may also be received via mail, police reports, hospital faxes, e-mails, or in-house referrals taken after office hours. You will have the opportunity to assist professionals in the community in obtaining a wide range of agency services, programs, and other community resources for consumers. This position is responsible for screening screens calls to identify presenting problems; acquiring and entering data pertaining to all daily activities; determining benefit eligibility; and assisting consumers with the completion of various forms, applications, and entitlements. These may include PACE/PACENET prescription coverage, property tax/rent rebate, LIHEAP (Low Income Home Energy Assistance Program), senior subsidized housing, LANTA van transportation, food stamps, PA Farmers' Market Produce Vouchers, as well as city and township tax rebates and exoneration forms. You may also be responsible for referring consumers to outside agencies for benefit checkups, enabling older adults to learn more about the federal, state, and local programs and benefits which they may be entitled to. Additional responsibilities include:

  • Interviewing consumers to determine eligibility for aging programs
  • Educating and providing consumers and family members with information about the agency's numerous services, programs, and community resources
  • Documenting all contacts made that includes a thorough and detailed case history, resolution, and/or disposition
  • Participating in senior community wellness fairs
  • Organizing and overseeing the coordination of holiday gift events

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year as an Aging Case Aide 1; or
  • Two years of public contact work, including one year in a human services program; or
  • Any equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Berks, Bucks, Carbon, Lehigh, Northampton, or Schuylkill counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
