Responsibilities

Brentwood Hospital is looking for a dynamic Patient Navigator to join our team of professionals who are dedicated to providing help and hope to individuals and families for the achievement of improving their mental health. The Patient Navigator manages the work-flow of the step-down process ensuring eligible patients are identified and engaged in discussion about outpatient services available at Brentwood Hospital prior to inpatient discharge. The step-down navigator will be responsible for data collection, communication and report generation to the involved departments and CEO.

ESSENTIAL JOB FUNCTIONS:

  1. Manages the workflow of the step-down process by ensuring eligible patients are identified and engaged in PHP/IOP/ECT discussion as a next step in treatment prior to inpatient discharge.
  2. Meets with every eligible patient to discuss the PHP/IOP/ECT or outpatient services, methods of transportation and to identify any obstacles that need to be resolved before discharge. This engagement will begin within 24 hours upon admission to the hospital.

  3. Submits data to the PI committee and periodically reviews step-down navigation for process improvement
  4. Independently plan and organize one's own schedule and activities.

Qualifications

  • Education: Associates degree in nursing from an accredited program of professional nursing or Master's degree in behavioral health related field required.
  • Experience: Previous mental health experience preferred;must be competent and comfortable in dealing with all aspects of the mental health field.
  • Licensure: RN or LPN, preferred
  • Hours: 8:30 AM - 5:00 PM, Monday - Friday
