What You'll Do: Empower Patients - Help individuals with diabetes access necessary resources such as medication assistance, insurance support, and follow-up care. Reduce Diabetes Burnout - Offer guidance and encouragement to help patients stay engaged in their care. Strengthen Community Connections - Connect patients with senior centers, local support groups, financial assistance programs, and food security resources. Collaborate with Healthcare Teams - Work alongside the endocrinology and iCMP care teams to develop a seamless bridge of care for patients. Qualifications: High School Diploma/GED required; Associate or Bachelor's degree preferred. Prior experience in healthcare, social work, or a related field preferred. Valid driver's license and ability to travel within the community as needed. What We're Looking For: Experience in community outreach, patient advocacy, or health coaching preferred. Knowledge of community resources and social services that support individuals with diabetes. Bilingual skills (Spanish or Haitian Creole preferred) to better serve our diverse patient population.
