Description
JOB PURPOSE: The Navigator position is a combination of duties including Outreach and Case Management. The person occupying this position will work directly with Veterans that are chronically homeless and assist them in accessing community support systems, services, benefits, and permanent housing. The Navigator directly supports the homeless Veteran community and collaborates with multiple service providers meeting regularly to triage, staff, and house the most vulnerable unsheltered homeless Veterans. The Navigator will be the lead on assisting the Veteran population with a history of homelessness. The Navigator will be the first point of contact with the affected veterans and building trust and rapport will be crucial for the role. The Navigator will have a caseload of approximately 20 Veterans and serve as the primary advocate for the Veteran for 1-2 years. Qualifications
ESSENTIAL JOB RESPONSIBILITIES:
ESSENTIAL QUALIFICATIONS: EDUCATION: Bachelor's Degree in Social Work, Sociology, Psychology or related field. EXPERIENCE: 1+ years case management experience; 3+ years preferred. Experience working with the homeless veteran population with outreach efforts targeting the most vulnerable, unsheltered homeless Veteran households preferred. Knowledge of VA Programs, facilities, and community preferred. ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion. LICENSES: LMSW, LBSW, LMFT preferred. Driver's License with clear record. VEHICLE: Must have daily use of a vehicle without prior notice. OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
