JOB PURPOSE:

The Program Coordinator will work within the Endeavors Transition Support Program (TSP) with the primary responsibility to include coordinating the matching process of connecting a transitioning service member to a registered Onward Ops mentor assigned to Endeavors and screening transition service members and connecting them to resources as needed. The Program Coordinator will be responsible for monitoring communication systems, sponsor availability and sponsor location. The Program Coordinator will track the progression of registration for new mentors and identify regions requiring focused outreach efforts. The Program Coordinator will coordinate and provide suicide prevention and intervention support to transitioning service members throughout various metropolitan areas.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:



  • Act as first contact point on behalf of Endeavors for mentors and transitioning service members. Provide initial tele-communications to welcome individuals to Endeavors
  • Work inside Onward Ops Google Workspace System to connect mentors with transitioning service members.
  • Monitor the influx of mentors to mentee ratios in multiple locations across Texas.
  • Create reports for data updates provided to leadership monthly.
  • Act as an interim mentor for individuals who are awaiting a mentor and/or identified transition service members needing additional support
  • Engage individuals interested in mentorship to complete the items needed for being an Onward Ops mentor.
  • Work with interns who are identified to support the Transition Support Program to increase service delivery.
  • Coordinate and provide evidence-based suicide prevention trainings to service members and sponsors.


  • Comply with policies and procedures governing the program and ensure that
  • the program is operating at maximum efficiency.
  • Under the direction of leadership collect and prepare data reports regarding
  • project operations by tracking Key Performance Indicators.
  • Create and utilize a resource guide for service coordination.
  • Provide brief suicide crisis intervention and crisis management as needed.
  • Coordinator community engagement opportunities for TSP clients to include
  • but not limited to; VA Enrollment Seminars, Resume Building Workshops,ect.
  • Support outreach events, as needed for awareness within the coverage areas of the program.
  • Assist in the development and implementation of a Pathway to Care process to connect individuals at risk of suicide to the appropriate level of care for the Transition Support Program.
  • Develop and maintain community network relationships with area of operations related to mental health and suicide risk.
  • Collaborate with multidisciplinary team to ensure available mentors for incoming transitioning service members.
  • Collaborate with a multi-disciplinary team as needed for project implementation and execution.
  • Demonstrate Exceptional customer service, in Everything you do, by placing the child, family, Veteran or client first to support our mission to "Empower people to build better lives for themselves, their families, and their communities."
  • Other duties as assigned.


ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor's degree preferred

EXPERIENCE: Preferred: At least two to three years' experience working in a behavioral health setting, suicide prevention, crisis intervention or with Veterans, Service Members, and their families. Familiarization with military culture, different branches of service, and military rank systems.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion. Employee is expected to report to work as scheduled, on time, and prepared to start work. This is a remote role requiring the employee to have the ability to remain on task in a work from home environment.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
