Job Overview:

AVP of Community Health

Job Details The AVP of Community Health reports to the Chief Operating Officer. The primary role of the AVP of Community Health is to direct and manage community engagement strategies & initiatives through collaboration with the CMMC Leadership Team & Public Relations. The AVP of Community Health will oversee the development and implementation of a community engagement plan strategically aligned to the intersection of business strategy, stakeholder engagement, and community needs. In this capacity, the AVP of Community Health is integrally involved in strategic planning and alignment of community engagement with key business priorities. This position has been designed to provide thought leadership and subject matter expertise to direct and align community engagement initiatives that will lead to Clara Maass Medical Center s positive community reputation & relevance in the RWJBH System. This position requires unique competencies, including systems thinking, orientation to learning, and collaboration. The AVP of Community Health must be adept at navigating complex and dynamic systems, connecting and building trust among multiple stakeholders, as well as adapting to dynamic environments.

Qualifications:

Required:

  • Bachelor s Degree required
  • Master s Degree preferred
  • More than 7 years in healthcare based marketing and community engagement

Preferred:

  • Previous experience in institutional community relations or corporate social responsibility is desired .

Essential Functions:

  • The AVP of Community Heath will have key managerial oversight of all Community outreach and community Health initiatives, the Center of Health for Latino Health and the Safe House Domestic Violence Center at CMMC.
  • The primary mission and goal of the AVP of Community Health, is to promote, create and sustain a culture of service excellence and patient satisfaction throughout the Medical Center and to develop, promote, and maintain community wellness outreach services according to the hospital s growth and community benefit agenda.
  • Identifies specific health needs, risk factors, and strategically constructs community
  • Health education programs with meaningful and measurable impact on long-term population health management metrics. Develops initiatives to target gaps in care and promote healthy living.
  • Establishes key partnerships with important community stakeholders.

Focuses on population health and encourages employee, staff and physician volunteerism through community health initiatives.

Community Health Needs Assessment (CHNA): sits on Community Health Steering Committee run by System EVP and ensures the completion of the state required CHNA. Holds focus groups to identify health priorities Primary Service Area.

  • Community Benefit Inventory for Social Accountability (CBISA): Oversees data collection for annual summary report to the community, the IRS 990 Schedule H, and CHNA and Implementation Strategy.
  • Evaluates impact in improving health of community.
  • Performs other duties as required
