TRIAGE NURSE (RN) - HOME CARE

Summary
Provides clinical support to the clinical services team by assisting in the coordination of the delivery of patient care services. Performs phone triage, data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care.

Job Duties

  • Triages patients through phone calls utilizing critical thinking skills and sound clinical judgement
  • Performs data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care
  • Provides hand-off communication to field staff, Clinical Specialist, and/or Patient Care Manager as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call.

Minimum Qualifications

  • Specialized Diploma NLN approved/accredited nursing program
  • 1 year nursing experience in a healthcare setting and
  • 1 year direct care nursing experience
  • Demonstrates knowledge and ability to care for patients based on the population served
  • Demonstrate critical thinking skills and sound clinical judgement
  • Demonstrates analytical and problem-solving skills
  • Ability to perform duties with accuracy and attention to detail
  • Demonstrates excellent verbal and written communication skills
  • Demonstrates excellent customer service skills
  • Demonstrates team behaviors and the ability to work independently
  • Demonstrates proficiency in basic computer skills
  • Demonstrates excellent time management, organization, and prioritization skills to manage a heavy workload
  • Ability to multitask and work with multiple interruptions
  • Ability to adapt to changing priorities
  • Demonstrates evidence of competency as per department/unit specific competency assessment criteria
  • Knowledge of agency and network clinical policies, procedures, and resource materials
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
  • DL - Driver's License_PA - State of Pennsylvania Upon Hire

Preferred Qualifications

  • Bachelor's Degree Nursing
