Job Description:

The Pathway of Hope Case Coordinator implements the social service aspect of the Pathway of Hope Case Management model by utilizing strength-based case management. This position provides resources and support to individuals to ensure the mission of The Salvation Army is effectively carried out.

What You Will Do:



  • Implement strengths-based and client-centered case management skills to identify client strengths, identify sufficiency goals and barriers to those goals, and implement strategies to achieve those goals
  • Develop and maintain working relations with community agencies
  • Work with Director of Social Services and Social Services department to identify potential clients and ensure clients' needs are met
  • Network with community agencies to develop and maintain external referral sources
  • Recruit and engage families interested in developing their level of sufficiency
  • Refer families to services designed to address identified goals, safety needs, and mental health issues
  • Support Pathway of Hope families by taking an active role in family functions when appropriate
  • Assist in the planning and implementation of Pathway of Hope activities including but not limited to classes, events, and graduation ceremonies
  • Work with staff to plan out and implement monthly family fun nights.
  • Complete all required documentation in SIMS (Salvation Army Information Management System) in an accurate, timely manner
  • Maintain client files per Pathway of Hope program guidelines
  • Meet with the Pathway of Hope Regional Coordinator regularly, implementing recommendations
  • Maintain and submit accurate monthly statistic reports per Salvation Army and United Way guidelines
  • Attend all required staff meetings, team meetings, and one-on-one meetings with appropriate staff members
  • Complete divisional, territorial, or local education forums and webinars as indicated by the Corps Officer, Director of Social Services, Pathway of Hope Regional Coordinator, and/or Divisional Social Services Director
  • Coordinate new holiday season adopt a family program
  • Other tasks as assigned by leadership



Minimum Qualifications: The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position.

Education: Bachelor's degree in social work, psychology, or related field required. You can change/add requirements based on job duties and budget guidelines.

Background Checks: Background Checks: Position requires a background check to be completed, and all background check results will be reviewed.

Experience: Minimum of one to two years' experience in social services and/or case management.

Certifications: Must have a valid driver's license and maintain The Salvation Army Driver's qualification standard; must complete Safe from Harm training within the first 90 days of employment; Caseworker Certification Program within 120 days of hire; SIMS data training within 30 days of hire.

Skills/Abilities:



  • Able to speak, write and understand English in a manner sufficient for effective communication with leadership, field personnel, and clientele
  • Computer proficiency with Microsoft products and ability to learn electronic reporting systems
  • Basic level knowledge of Spanish is helpful
  • Strong organizational skills
  • Ability to work effectively with individuals from diverse cultures and socioeconomic backgrounds.


  • Provide guidance and promote the spiritual dimension of Salvation Army social services programs, emphasizing the importance of Christian influence and evangelism in all Salvation Army ministries, including delivery of social services in contemporary settings.


Supervisory Responsibility: This position may supervise volunteers working in a mentoring relationship with clients.

Physical Requirements Include:



  • Good speaking, hearing and vision ability, and excellent manual dexterity
  • Lifting, pulling, and pushing materials up to twenty-five pounds

  • May require standing for extended periods


Travel: Travel may be required to adequately address the needs of Pathway of Hope clients.

Working Conditions: Work is performed in an office and community center environment; May require weekend and evening work.
