Aftercare Case Management Specialist

Cornerstones of Care is a mental and behavioral health nonprofit certified in trauma-informed care that provides evidence-based prevention, intervention, treatment, and support services to help children and families improve their safety and health by making positive changes in their lives. Each year, our team empowers children and families in Kansas, Missouri, and beyond through three key service areas:

• Youth & Family Support - We help youth gain independence through social and living support programs while empowering families with the skills and resources they need to become resilient and successful.
• Foster Care & Adoption - We reunify and unite families while recruiting and providing support to foster parents and youth in foster care.
• Education & Community Trainings - We help students achieve academic success while giving educators the tools to create safe learning environments to improve their students' behaviors and offer innovative learning opportunities to build and improve knowledge in the community.

We are seeking an Aftercare Case Management Specialist - Residential - to join our team. Your role will carry out case management services as required by the contract and licensing regulations.

As a member of the Residential team, you will work with other team members and report to our Program Manager of Aftercare.

WHAT YOU WILL DO:

• Exercise discretionary judgment in making decisions on the management and delivery of services to clients.
• May attend various meetings and/or court hearings and facilitate groups as necessary.
• Data entry into various software programs.
• Crisis intervention
• Work with therapist as team in coordinating services for clients and their families
• Complete Date Collection and Client Satisfaction
• Utilize community resources
• May be required to travel to attend meetings with assigned clients

WHAT YOU WILL BRING:

Our ideal candidate will have 1-3 years of relevant experience and the following:

Bachelor's degree in human behavioral science (which includes 30 semester or 45 quarter hours either in development of human behavior, child development, family intervention techniques, diagnostic measures or therapeutic techniques, such as social work, psychology, sociology, guidance & counseling and child development).

Must possess, or be willing to attain CPR, First Aid, and Safe Crisis Management (SCM) Training (with sufficient passing score) within 2 weeks of hire date.

• At least 21 years of age and pass background check, physical and drug screening
• A valid driver's license, proof of current vehicle insurance and reliable transportation

Starting Salary: $43,888 - $47,000 (Annually)
