The Care Coordinator is responsible for providing education and support to members with SUD issues to promote improved health outcomes and facilitate their navigation of the healthcare system. The Care Coordinator role necessitates strict adherence to confidentiality guidelines specified in the UHG Employee Handbook. Responsibilities include conducting screenings upon disclosure of SUD, serves as a community liaison, and fosters relationships with key community figures to advocate for improved health, social, and environmental conditions for RMHP members. The Care Coordinator conducts member assessments and documents responses to care coordination interventions made. They attempt to engage resistant, challenging and non-compliant members and offer supports when needed. Duties also include establishing and maintaining outcome reporting systems, communicating member issues to relevant departments, participating in staff meetings, case conferences, and in-services. The Care Coordinator conducts follow-ups with members after providing resources, engages in daily outreach to Inpatient Mental Health facilities, assists members in locating treatment options and placements. They also participate in Physical Health Bed Rounds and SUD facility meetings to discuss resources available for members. The Care Coordinator oversees the Contingency Management Program, attends Perinatal PCOC Advisory Meetings, provides guidance and trainings to Peer Support Specialists, and cultivates relationships with SUD providers. Furthermore, they liaise with Withdrawal Management facilities, update documentation in member campaigns, and ensure accurate and timely completion of all necessary documentation. This position entails travel and reliable transportation, with the employee meeting Colorado licensure and company liability insurance prerequisites.

Primary Responsibilities:



  • Educate and assist members with SUD issues to promote improved health outcomes and facilitate their navigation of the healthcare system
  • Maintains confidentiality and respect confidences as outlined in the UHG Employee Handbook, a critical and essential component of this position
  • Completes screeners when SUD is disclosed upon reaching the member
  • Serves as a community liaison, and fosters relationships with key community figures to advocate for improved health, social, and environmental conditions for members through coordinated linkages or referrals
  • Conducts member assessments, including a. Assessing the changing needs and conditions of members and communicating this information to all involved Care Coordinators, community partners, physicians, and other appropriate individuals according to department policies and procedures. b. Documenting assessments and client/family responses to care coordination interventions at the time of the encounter, meeting departmental standards and deadlines for timely completion of all required documentation, and adhering to current agency productivity standards
  • Attempts to engage resistant, challenging and non-compliant members and offer supports when needed Develop and maintain a reporting system for outcomes
  • Communicates member issues requiring interventions to appropriate departments and providers
  • Participates in staff meetings, case conferences, and in-services, maintaining familiarity with all policies and procedures that impact decisions and care
  • Follows up with members after providing resources to offer additional support
  • Performs daily outreach to Inpatient Mental Health (IPMH) facilities via email to connect with members, obtain signed Releases of Information (ROIs), and engage with members regarding SUD needs, supporting them in accessing treatment
  • Assists facilities with SUD referrals
  • Participates in Physical Health Bed Rounding for members with primary physical health needs
  • Participates in SUD facility meetings and offer additional member support
  • Manages the Contingency Management Program by: a. Verifying referrals to the program and ensuring eligibility. b. Conducting outreach emails to members. c. Distributing Contingency Management rewards to members. d. Executing biannual data projects to promote program efficacy and analyzing the need for changes to reward requirements
  • Attends Perinatal PCOC Advisory Meetings bi-monthly
  • Provides training and support to Peer Support Specialists
  • Builds and maintains relationships with SUD providers to better support members
  • Corresponds with Withdrawal Management (WM) facilities for member updates regarding admission and discharge to support successful discharge planning
  • Updates documentation in member campaigns upon routing or changes in level of care/facility
  • This position requires travel, necessitating independent, reliable, flexible, and on-demand transportation at the incumbent's expense for travel between various locations and timely arrival and departure. If the employee chooses to satisfy this requirement by driving a vehicle, they must meet the requirements for Colorado licensure and company liability insurance coverage

Required Qualifications:



  • 3+ years of experience working within the community health setting in a health care role
  • 2+ years of progressive related experience working with diverse populations, community or faith-based organizations
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Experience in care coordination and/or care management
  • Knowledge of health education, motivational strategies, and an empathetic manner working with the underserved

  • Bilingual in English/Spanish

The hourly range for this role is $23.70 to $46.35 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
