Key Responsibilities to include but not limited to:
*Supervision & Mentorship: Provide leadership, training, orientation, and professional development for social work staff, including LCSWs and interns. *Clinical Assessments & Interventions: Conduct psychosocial assessments and develop individualized treatment plans for inpatients, outpatients, and emergency department patients. *Crisis Management: Perform safety assessments, including mandated abuse reporting (child, disabled adult, elder) and intervention strategies. *Care Coordination: Plan and collaborate with interdiscliplinary teams, and ensure seamless transitions of care. *Regulatory Compliance: Maintain thorough documentation in alignment with HIPAA, Joint Commission standards, and Centers for Medicare/Medicaid regulations. *Patient Advocacy: Serve as a key liaison between patients, families, and healthcare providers to promote well-being and resource accessibility.
