City/State Chesapeake, VA Work Shift Rotating Overview: Overview Evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients . Record the outcome of calls and make follow up calls as necessary. Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Act as a resource for patient and referral source issues. Education Highschool Graduate or Equivalent Certification/Licensure LPN Experience Minimum 1 years of experience with clinical and customer service in a healthcare environment We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$17.31 - $28.85. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
