Responsibilities
Foundations San Francisco Intensive Outpatient Program (IOP) is looking for a dynamic and talented Case Manager/SUD-MH Counselor. Located in the heart of the Embarcadero district, Foundations San Francisco offers comprehensive, integrated outpatient treatment in a convenient urban setting. Flexible daytime and evening programs address addiction and mental health issues in a supportive atmosphere. The Case Manager/SUD-MH Counselor tracks clients from the intake call through the admissions process and episode of care including general oversight of case management, aftercare planning, and discharge. The position provides primary interfacing with referents, family members, and the clinical team. Incumbent works closely with the Supervisor Patient Care Coordinator and Director to ensure smooth day-to-day operations and may fill in as group facilitator or assessor when necessary. Incumbent represents the facility as dictated by the company's policies and procedures and under the direct supervision of the Director of Outpatient Services; also works closely with the Director of Business Development. Key responsibilities include: * Responsible for smooth and expedient admission of clients into the facility and ensures treatment is aligned with best practices and standards of care. Qualifications Position Requirements: * Education: High School Diploma or GED with associate degree or bachelor's degree strongly preferred.
