This position is responsible for the efficient processing of all incoming referrals which includes the coordination of information flow from Hospital Case Managers, Physician offices, and Careport to clinical teams.

I. Major Responsibilities:
1. Processes all incoming referral request according to departmental procedures and policies. Researches referral requests as needed to ensure all intake information is completed. Refers unusual referral requests to the Clinical Manager or designee according to departmental policy to resolve.
2. Enters referral information into the electronic medical records in a complete and timely fashion. Gathers missing information and enters into the system.
3. Scans and saves regular required referral information.
4. Reviews and completes multiple assigned work queues on a daily basis.
5. Participates in daily huddles with the Clinical and Scheduling teams to review pending referral requests from all referral sources. Prepares reports of pending referral requests for review by the team to determine available service options. Provides referral sources and patient's families with correct information regarding the service to be provided whether internally or through another agency.
6. Verifies insurance eligibility and authorizations are complete, timely, and accurate according to departmental policies to help ensure patient is cleared for services. Documents insurance coverage. Refers nonstandard insurance questions to the Manager.
7. Responds to callers' inquiries and communicates accurate and complete information. Keeps referral sources and/or patient's families informed throughout the process.
8. Communicates with a wide variety of referral sources, both internal and external customers including current and prospective patients.
9. Schedules initial patient visit and ensures communication with Home Health Team Schedulers.
10. Verifies what prior hospice services, if any, the patient has received to understand where the patient is in the hospice benefit period and documents the information obtained from the Medicare system.
11. Communicates the durable medical equipment (DME) and/or supplies needed for the patient to the Home Health Team Schedulers to ensure the appropriate equipment and/or supplies are delivered on time to the patient.
12. Maintain knowledge of current laws and regulations of federal, state, and regulatory bodies relative to home health and hospice and works within those constraints to maintain actions within the agencies' and corporation's compliance programs.
13. May provide input to department policies and procedures.
14. May assist with orientation of new Caregivers to the department.

License/Certification/Education:

1. High School diploma or equivalent

Experience/Skills:
Required:
1. Minimum two (2) years of office experience and one (1) year of Intake experience, preferably in a health care environment
2. A comprehensive understanding of Medicare PDGM guidelines
3. Strong computer/data entry skills
4. Excellent organizational/communication skills
5. Ability to handle multiple tasks
6. Adheres to Agency's confidentiality policies
7. Excellent customer service skills
8. Ability to follow up to completion on all duties and team responsibilities

Preferred:
1. Medical terminology
2. Epic experience
3. ICD-10 coding knowledge

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
