Position Summary: The Team Manager is accountable for the performance of the team, by ensuring that all performance metrics are met as well as overseeing the team's daily operations, including the member experience, care management, and care coordination. The Team Manager is primarily responsible for ensuring that their team is providing high quality, regulatory compliant, longitudinal care coordination and care management, to a dedicated panel of CCA members. The Team Manager is expected to hold their team accountable and lead the team to be successful in achieving key performance indicators (KPIs) as set forth by CCA leadership. Additionally, the Team Manager is expected to monitor team performance from a quantitative and qualitative perspective as it relates to process compliance, evidence based standards and member outcomes. The Team Manager is expected to participate in the workflow development and/or process improvement initiatives as instructed. The Team Manager reports to the Clinical Director. Supervision Exercised: Yes, which includes but is not limited to providing leadership, work direction, and performance feedback to subordinate staff. Essential Duties & Responsibilities:
- Responsible for leading a care management and care coordination team.
- Monitors and reports performance related key performance indicators (KPIs) to ensure clear visibility into the team's operational and outcome performance excellence, regulatory compliance and achievement of performance goals.
- Leads with a strong emphasis on data management.
- Cultivates and maintains collaborative relationships.
- Create a culture of curiosity, problem solving, and continuous learning. Encourage Root Cause Analysis (RCA) and 5-Why's when appropriate to fully understand how to overcome challenges.
- Lead by example with strong motivational interviewing, time management, emotional intelligence, conflict management, and changement management skills.
- Hold direct reports accountable for performance, developing, coaching and/or implementing disciplinary actions as needed.
- Participate in hiring process for team.
- Participate in workflow development and/or enhancement initiatives as requested.
Working Conditions:
- Standard office conditions.
- Work locations include residential and community sites.
Required Education (must have):
- Associate's Degree or 2+ years of applicable experience.
Desired Education (nice to have):
- Bachelor's Degree or equivalent experience
Required Licensing (must have):
- Valid driver's license with ability to travel to CCA sites across the state of Massachusetts and to visits in a home, residential, or provider-based setting.
Required Experience (must have):
- 2+ years with a health plan, medical group or health-related facility such as a hospital or outpatient center
- Experience with cross-functional collaborative process improvement
Desired Experience (nice to have):
- 5+ years of related clinical experience
- 1+ years of leadership experience

- Must be familiar with using a laptop computer as well as Windows processing systems and have knowledge of Microsoft Office, specifically Word, Excel, and Outlook.
- Able to support teams with utilization of telehealth technologies (video, chat, etc.), when appropriate, as an approved modality for a variety of clinical care and care management services
