Location: Medical Center Office Building (901 7th Ave)Department: Homeless InitiativeShift: First Shift (United States of America)Standard Weekly Hours: 4Summary: Utilizing a collaborative process, the RN Case Manager will assess, coordinate, and implement a medical plan of care for children residing in selected Tarrant County homeless shelters. This individual will be charged with the development and coordination of a program which oversees the health care for designated homeless children treated in the Cook Children's Health Care System. This position will be responsible for the compilation of data to measure the impact of this program on homeless children and the possible justification for replication in other communities. This individual will perform an ongoing review of the health care management of inpatient homeless children and follow up to monitor compliance with the post discharge plan of care. Hours: PRN First shift *Will require Full-time training in Fort Worth the first 8-12 weeks Education/Experience:
