Probationary period is four (4) months for full-time employees, and six (6) months for part-time employees. The NP participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills. The NP demonstrates core and unit specific competencies based on the patient population served. The Nurse Practitioner will assess and provide care to a selected group of patients in collaboration with physicians. The NP will be responsible for carrying out the plan of care for his/her patient panel. The NP contributes to the actualization of the philosophy and objectives of the Geriatrics Division/Department of Medicine. The NP practices within the scope of practice in care delivery. The NP participates in performance improvement and relevant research activities, and maintains professional practice standards and clinical expertise. The Division of Geriatrics at Montefiore Medical Center-Albert Einstein College of Medicine, Bronx, New York is seeking a full-time Nurse Practitioner to join our faculty. The Division of Geriatrics includes 13 physician faculty, a Geriatrics Social Worker , a highly regarded fellowship program, outstanding clinical programs in all care settings, and a nationally recognized research infrastructure. The clinical responsibilities are in the ambulatory settings. The ambulatory role includes sessions in our geriatrics ambulatory practice where you would provide primary care and consultative service to a panel of patients 65 years and older. There are numerous opportunities for academic growth through involvement in program development and educational initiatives. The NP will work under the supervision of a Geriatrician when necessary to manage more complex patients. This position is 100% grant funded; Responsibilities(include but not limited to): * Perform comprehensive geriatrics assessments including assessment of cognitive impairment, functional status and goals of care * Develop comprehensive management plans with attention to patient specific needs or preferences * Document medical encounters in the electronic medical record system * Provide longitudinal care for a panel of patients under the supervision of a Geriatrician * Collaborate with an interdisciplinary team including nurses, social workers, physicians, and physicians in training to provide high quality clinical care EDUCATION, TRAINING & EXPERIENCE: New York State License for a Registered Professional Nurse Current New York State Nurse Practitioner specialty certification and registration. Masters or post Masters Certificate in Geriatrics as a nurse practitioner preferred and the Director of Nursing and/or Chairman of the Department, or Division Director. National certification in nurse practitioner specialty (ie: ANCC, AACP or NAPNAP) preferred. Spanish language proficiency is an added benefit but not required.
