Nurse Case Manager/Residential RN - contract

FlexStaff is seeking a Nurse Case Manager/Staff Nurse for a 13 week contract with our client, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.

This position will be covering residential group homes in Yonkers, Garrison, Carmel

Schedule: Monday-Friday, 9am-5pm

Contract Pay Rate: $80/hr

The Nurse Case Manager will coordinate the delivery of health care services for persons receiving services residing in a residential facility.

Responsibilities

• Coordinates the delivery of routine and specialty health care services for adults with disabilities residing in residential programs
• Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated. Maintains health care profiles.
• Provides guidance and training to direct care staff to ensure proper delivery of health care services.
• Develops plan of nursing services and oversees the implementation of the health care plan by educating person's receiving services, DSP/Caregivers and families in the plan of nursing services.
• Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.
• Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers
• Completes relevant medical documents, assists in developing policies, procedures and forms; and establishing working systems.

Requirements

• NYS RN License, BSN preferred.
• Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
• Excellent oral and written communication skills..
