Overview

Oversight of the assigned care management line of business. Ensure staff are providing services in accordance with care management service requirements set by the state and company. Responsible for reviewing all Tailored Care Management care plans and Individual Support Plans (ISPs) and will provide guidance to care managers on how to meet members' needs. Supervises up to eight Care Managers.



Responsibilities

    Recruit, hires, trains, and supervises the care management staff, being responsive and accountable to the needs of staff and enrollee/family/guardian.
  • Responsible for direct supervision of case management team as well as coordination, accuracy, appropriateness and timeliness of workflow and caseloads.
  • Reviews all Care Management Plans and Individuals Service Plans (ISP) to ensure they meet enrollee needs and technical requirements including timeframes.
  • Provides guidance to care management staff on how to meet enrollees' needs through care coordination.
  • Support the care management team in developing relationships and connections that will help enrollees in achieving whole person care outcomes.
  • Ensures systems are in place to secure, maintain and retain care management records.
  • Supervises and manages the care management staff, being responsive and accountable to the needs of enrollee/family/guardian and other stakeholders.
  • Creates and organizes all Care Manager and Extender schedules to ensure that all services, activities, and interventions are staffed appropriately to maintain proper coverage and completion of the department's goals.
  • Provides educational updates for care management staff on an ongoing basis, including daily huddles.
  • Coordinates with regional supports, such as Human Resources, Business, Quality, etc. for support, resources, and enhancement of the care management line of business.
  • Communicates with all entities in a professional manner to promote an environment of understanding, respect, and commonality.
  • Reports issues of concern, general departmental activities and staffing needs to the Executive Director.
  • Ensures the care management team conducts activities that lead to billable services and bills services at required intervals, leading to a balanced budget.
  • Utilization of electronic systems as needed.
  • Attends industry functions to promote positive relationships with stakeholders.
  • Provides coaching, reinforcement of positive work behavior, in-services, and resources to improve clinical skills and communication with staff, I/DD enrollee/family/guardian, and all entities involved in the program.
  • Promotes resolutions to conflicts, employee issues, disputes, disruptions in services, and unprofessional behavior of the care management staff.
  • Evaluates performance of the care management team members.
  • Heads quality improvement and measurement activities in conjunction with the Executive Director, achieving identified targets and outcomes.
  • Completes other duties as assigned.


Qualifications

Qualifications:

  • Three or more years of supervisory experience in healthcare. Ability to supervise and direct the work of others.
  • Three to five years' experience providing care management, case management, or care coordination to complex individuals with an I/DD or a TBI, depending on education.
  • Ability to perform work with a high degree of quality and autonomy.
  • Must meet all agency requirements for pre-employment and those required by the state of NC.

Education:

  • A bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area and five years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI; or

A master's degree in a field related to health, psychology, sociology, social work (e.g., LCSW), nursing, or another relevant human services area, and three years of experience providing care management, case management, or care coordination to complex individuals with an I/DD or a TBI.
