Job Description

General Purpose of Job:

Responsible for assessing, planning, implementing, and evaluating patients in collaboration with the hospice and palliative care physicians.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

Assesses, plans, treats, and evaluates care for patients with palliative care-based needs.
Collaborates with health care team and coordinates care for patients.
Organizes and conducts patient/family case conferences.
Conducts informed consents with patients considering Hospice Care.
Works closely with physicians, associates, patients, and families to provide quality care to patients with serious or life-limiting illness.
Attends Interdisciplinary Team meetings.

Job Requirements

General Requirements	* Current State of Michigan registered nurse license* * Current State of Michigan certified nurse practitioner license
Work Experience	* Minimum of two years experience with either palliative care, hospice, critical care, case management, or oncology.
Education	* Masters of Science in Nursing
Specialized Knowledge and Skills	* Must have strong interpersonal, communication, and problem solving skills * Must be well organized and able to manage time effectively

The University of Michigan Health - Sparrow Lansing is seeking an enthusiastic and dedicated part-time PA/NP to join their Palliative and Hospice team . The University of Michigan Health - Sparrow delivers exceptional quality care in a diverse and family-friendly community and is committed to improving access to medical care across the Mid-Michigan region.

- 28 hours per week
- Clinic and inpatient
- Call 5- 6 weekends per year
- Preferred 1 year experience in Hospice and/or Palliative Care
- EPIC EMR
- Top performer in quality initiatives
