The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. The Clinical Navigator helps patients negotiate the healthcare system, especially those facing complex and chronic medical conditions or navigating challenging healthcare environments. They provide education, coordinate care, address barriers to adherence, offer emotional support, and assist with retention in treatment. The CN serves as a guide and advocate for patients, ensuring they receive the necessary support and resources to access and manage their healthcare effectively. This person often acts as a liaison between medical staff and patients to ensure continuity of care, enhanced patient experience and improved treatment outcomes. The CN facilitates communication and coordination between patients, healthcare providers, and pharmacies to ensure patients receive and properly utilize prescribed medications. This role focuses primarily on improving medication adherence and access, often involving tasks like prescription processing, insurance verification, and patient education. Bridging the gap between healthcare providers, pharmacies and patients, facilitates effective medication management and promotes patient adherence to prescribed treatment plans. Responsibilities would include tasks such as: Communication and Coordination: Serving as a point of contact between patients, doctors, pharmacies and other interdisciplinary care team members to clarify medication instructions, resolve issues, and ensure smooth transitions between care settings. Medication Access: Assisting patients with eliminating barriers to prescription processing, accessing refills, facilitating prior authorizations, and navigating adequate insurance coverage to obtain prescribed medications, potentially including financial assistance programs. Ensuring timely processing and coordinating delivery of medications in alignment with appointment schedules. Patient Education and Support: Providing patients with information about their medications, including dosage instructions, potential side effects, and adherence strategies. Relationship Management: Establishing and maintaining relationships with healthcare professionals, pharmacies, pharmaceutical company representatives and insurance providers to optimize patient care and streamline medication-related processes. Compliance and Documentation: Adhering to relevant regulations and standards, maintaining accurate records, and documenting patient interactions and medication information. Specific examples of tasks might include:
Managing prior authorization requests for specialty medications with insurance companies and other payers. Completing associated paperwork and communicating with all parties throughout the process to obtain approval for medications requiring pre-authorization. Assist physicians in insurance peer-to- peer reviews; providing supporting documents and sitting in on conference calls.
Managing prescription refill requests, ensuring timely processing and delivery of medications.
Identifying and assisting patients in applying for medication assistance programs offered by pharmaceutical companies or other appropriate organizations.
Tracking medication usage and proactively addressing any adherence issues or concerns.
Coordinating patient appointments for medication administration or follow-up visits.
Ensuring accurate and timely documentation of medication-related information in patient charts.
Providing education and training to other healthcare professionals and/or patients on diagnosis and medication-related topics. Knowledge, Skills and Abilities required:
