Job Details The Philadelphia TelePrEP Program (PTP) is a joint initiative between Jefferson Einstein Philadelphia and the Philadelphia Department of Public Health/Division of HIV Health (PDPH/DHH) to provide at-home and telehealth-based HIV prevention services, including during untraditional clinic hours (monthly evenings and weekends). The PTP Supervisor is responsible for a full range of program implementation and management activities.
Job Description
- Oversee delivery of services
- Manage program workplan and deliverables
- Optimize schedule utilization to meet/exceed capacity and visit benchmarks
- Collaborate with PTP medical director, physician, and IDC program director on program monitoring, strategic planning, and capacity-building
- Liaise and collaborate with the testing providers and PDPH/DHH marketing vendor
- Coordinate efforts to recruit and retain patients in HIV prevention services
- Coordinate professional education and provider-level training on telehealth-based screening, assessment, and prescription of PrEP and nPEP at least annually
- Assist patients in linkage and navigation of the healthcare system: insurance enrollment and co-pay assistance enrollment, linkage to primary care, mental health, substance use, and housing programs;
- Iterate program's resource and referral guide regularly based on new resources, staff experience, feedback, and input
- Be available and responsive to program staff, including during scheduled untraditional business hours
- Track unreimbursed services and arrange payment from grant or other funds
- Lead PTP team meetings, case conferences, and facilitate professional development opportunities for program staff
- *Represent PTP program internally and externally; present on program independently and in conjunction with PDPH/DHH
- Coordinate program schedules, staff coverage and time off
- PTP Management and Leadership:
- Staff hiring, supervision, performance evaluation, and personnel management
- Monitor adherence to program workflows and update or re-train as needed
- Actively participate in PDPH/DHH meetings, learning collaboratives, health equity initiatives, and EHE activities;
- Serve as primary point of contact between Jefferson Einstein Philadelphia and PDPH/DHH;
- Collaborate with community partners
- Solicit feedback from patients and community members on PTP services and implement improvements in response to patient input
- Enhance positive healthcare outcomes and community awareness about HIV/AIDS, impacting community health through education, community outreach and engagement, linkage, referrals and direct service delivery
- Identify and evaluate models that transform sporadic users of health care into regular and continual users of healthcare, and identify effective methods to support and retain clients in care
- Promptly attend staff meetings, in-services and trainings
- PTP Reporting, Evaluation, and Continuous Quality Improvement:
- * Data collection, outcome measurement, monitoring, and reporting
- * Track progress using CareWARE, EMR, and MS excel as appropriate. Use data to identify needs assessment and conduct quality improvement activities
- Ensure all required reports are submitted completely and on time
- Collaborate on program evaluation/research; maintain current CITI training and conflict of interest/financial disclosures
- Author conference abstracts on program and present on program when accepted
- Disseminate program results and best practices
- Other projects/duties as assigned
Minimum Education and Experience Requirements:
- Master's degree in public health, social work, or health administration or the equivalent years of relevant work experience (required)
- Experience in project management and healthcare services coordination.
- Experience working on health projects serving marginalized populations.
- Experience providing staff supervision or personnel management.
- Experience working on HIV, sexual health promotion, and grant-funded initiatives .
