Job Details

The Director of the Wyss Institute at Jefferson Primary Care is responsible for the implementation of the 5-year Wyss Institute program deliverables designed to provide comprehensive clinical and social needs supports for Jefferson Health patients in NE Philadelphia. Key areas of focus including, but are not necessarily limited to, the following:
-In partnership with director of the institute, regional administrative leaders and Jefferson Real Estate, finalize locations, develop workplans and ensure completion of all capital improvements to Jefferson care sites in service to improved care experiences for patients.
-Lead completion of needs assessment to inform key priorities for expanding interprofessional care teams at Jefferson Primary Care sites to enhance comprehensive, person-centered care to address the medical and social needs of patients with an emphasis on addressing the unique needs of target populations.
-In partnership with Real Estate and regional operational, clinical and administrative leaders, provide project management support to support timely completion and opening of new Jefferson Primary Care location NE Philadelphia. Support and provide project management to design and implement unique Wyss Institute care model to address unmet need for primary care services in the area. Primary Care site will include the unique assets and resources of the Wyss Care Model to support the needs of all patients which specific training on supporting the unique needs of target populations.
-Establish comprehensive, community-based Wyss Support Network to be leveraged across Jefferson's large clinical network and integrated in community-based settings in NE Philadelphia to address the unique needs of NE Philadelphia communities with an emphasis on supporting the specific needs of target populations.
-Track and regularly report on progress and outcomes measures for Wyss Institute project.

Job Description

Essential Functions:
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Design, execute, and ensure completion of complex, multi-faceted workplans for developing entirely new systems of care for patients across multiple departments and stakeholders.
  • Develop and steward relationships with community-based organizations to develop
  • Provide detailed, independent project management to a wide range of projects with a wide range of complexity and system stakeholders.
  • Develop and disseminate progress and outcomes reports appropriate for a wide range of audiences.
  • Lead project teams and direct reports as assigned.

Minimum Education and Experience Requirements:

Education:

Bachelor's degree: Master's degree preferred.

AND

Experience:

Seven years of relevant experience in complex clinical program management and team management.
