The Case Management and Social Work team at UVM Medical Center is hiring two Trauma Case Managers to join their team!

Please Note: These positions are located onsite at UVM Medical Center. We are currently hiring for two roles: one part-time position (16 hours per week) and one full-time position (40 hours per week).

JOB DESCRIPTION:

The Trauma Case Manager (CM) plays a key role in guiding patients through recovery after acute physical trauma. They coordinate care across all stages-from emergency treatment to rehabilitation and discharge-ensuring timely, efficient, and patient-centered support. Collaborating with interdisciplinary teams, the CM helps improve clinical outcomes, manage resources, and address recovery barriers such as mental health, substance use, housing instability, and other biopsychosocial challenges. Acting as a central liaison among patients, families, providers, payers, and community services, the CM promotes trauma-informed care that fosters healing, resilience, and long-term well-being.

CORE COMPETENCIES & SKILLS:

1. Clinical Expertise

  • Demonstrates knowledge of trauma care protocols and recovery pathways.

  • Assesses patient needs and develops individualized care plans.

  • Monitors clinical progress and adjusts interventions accordingly.

2. Care Coordination

  • Facilitates seamless transitions across care settings (ED, inpatient, rehab, outpatient).

  • Collaborates with interdisciplinary teams to ensure continuity and quality of care.

  • Manages referrals to specialists, community services, and support programs.

3. Communication & Collaboration

  • Serves as a liaison among patients, families, providers, payers, and community resources.

  • Communicates effectively to ensure shared understanding of care goals and plans.

  • Advocates for trauma-informed approaches that promote healing and resilience.

4. Psychosocial Assessment & Support

  • Identifies and addresses barriers to recovery, including mental health, substance use, housing instability, and social determinants of health.

  • Provides emotional support and crisis intervention as needed.

  • Connects patients with behavioral health and social services.

5. Resource Management

  • Utilizes healthcare resources efficiently to optimize outcomes and reduce costs.

  • Navigates insurance and reimbursement processes to support patient access to care.

  • Tracks and reports utilization metrics and outcomes.

6. Cultural Competency & Equity

  • Applies culturally sensitive practices in patient interactions and care planning.

  • Promotes equitable access to services and supports for diverse populations.

EDUCATION:

Master's in social work from an accredited school of social work. LICSW preferred.

OR

Current RN licensure or compact licensure recognized by the State of Vermont is required.

OR

Master's degree from an accredited institution in a Health or Human Services field. Licensure preferred.

OR

Bachelor's of Science or higher degree from an Accredited Occupational Therapy Program. Licensed or eligible to be licensed to practice occupational therapy in the State of Vermont.

OR

Bachelor's of Science or higher degree from an Accredited Physical Therapy program. Licensed or eligible to be licensed to practice physical therapy in the State of Vermont.

EXPERIENCE:

Minimum of (3) years experience in a clinical setting, including (2) years in a medical setting, preferably a hospital. (2) years case management experience or a similar role preferred.
