The Washington County Developmental Disabilities Division (WCDDD) is recruiting for Bilingual and Non-bilingual Developmental Disabilities Services Coordinator positions. You can learn more about our division at: Developmental Disabilities About the Position: This position is responsible of the coordination of services for adults and children with developmental disabilities who live on their own, with their families, or in a foster care or group home setting. This includes conducting assessments, developing and facilitating service plans, monitoring of plan-related services, and resource referrals. This position may be eligible for an alternative and/or hybrid work schedule.
Bilingual Positions: Designated bilingual positions require that the incumbent have knowledge of another language (primarily Spanish) besides English. It is required to possess precise writing skills (including spelling, grammar, and punctuation), fluent speaking skills, and the ability to communicate clearly and concisely both orally and in writing. If you possess these skills, you are encouraged to apply.
Upon successfully passing the applicable proficiency assessment test in a selected language, positions that are eligible for the bilingual differential pay will offer in an amount of 5% of the base pay rate, or a flat rate of seventy-five ($75) per pay period, whichever is greater. Pay periods are every two weeks.
Who we are looking for:
The ideal candidate will have a combination of lived and/or professional experience in the Human Services fields, with an understanding of working with Developmental Disabilities Services. This could include caseload management, assessment procedures, team coordination, and effective report and plan writing. This position requires the ability to collaborate with a variety of communities and diverse life experiences through a variety of different mediums; including but not limited to: telephone, email, mail, face to face in person or in a virtual setting. Additionally, candidates will need to be comfortable with travel and working in various settings including family homes, foster homes, and group homes and be able to work with a variety of individuals and their support teams.

ESSENTIAL JOB DUTIES

Developmental Disabilities Services Coordinator is a working title. This position is classified as a Mental Health Services Coordinator II. Classification descriptions are typically written broadly. To review this classification, please use this link: Mental Health Services Coordinator II - Developmental Disabilities. Primary duties may include, but are not limited to:
- Conduct all monitoring and site visits per the divisions policy and procedures that are completed monthly, quarterly or yearly per Oregon Administrative Rules (OAR) requirements throughout the plan year.
- Complete unduplicated Qualified Case Management Contacts (CMCs) quarterly or monthly activities for individuals receiving K-plan and/or Waiver services. Complete all progress notes, including Targeted Case Management, Waiver Case Management and CMC's timely according to the divisions policy and procedures.
- Complete plan development per division policy and procedure and OAR in the form of an annual plan, individual support plan, or other required format. Plan activities may include, but are not limited to participation in assessment activities, completing supplemental documentation, providing Notification of Rights, providing WCDDD Privacy Practices, obtaining needed Releases of Information, offering voter registration when an individual turns 18, conducting required meetings and obtaining needed signatures. All needed documentation will be completed per division policy and procedure and OAR.
- Follow up on serious incidents, complete needed protective service activities, and complete Centralized Abuse Management (CAM) entries per division policy and procedure and OAR. Secondary-Demonstrate an understanding of the Community Developmental Disability Program (CDDP), Oregon Revised Statute (ORS) and Case Management Services OARs, especially OARs relevant to services being provided to individuals on their caseload.
- Assist individuals and families to understand financial, medical and other benefits and provide information and referral to community resources as needed.
- Share information obtained regarding service delivery resources and options with other DD team members and their supervisor.
- Monitor the quality of services delivered within the county and bring any concerns to the attention of their supervisor. Report identified insufficient service delivery resources or options to their supervisor.
- Update information in the Electronic Medical Records System (EMR) at least annually and as significant changes occur.
- Terminate services and close files following Oregon Department of Human Services (ODHS) Notice of Planned Action requirements, the division policies and procedures and OAR.

MINIMUM QUALIFICATIONS

Education and Experience Must meet one of the following minimum qualifications to be considered:
- Bachelor's degree in any field; AND One (1) year of human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing;
OR - Associate's degree in a behavioral science, social science, or a closely related field; AND Three (3) years of human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing;
OR - Five (5) years of human services related experience, such as work providing assistance to people and groups with economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing.

Additional Requirements:
- Must have the ability to arrange for timely transportation in order to attend appointments and meetings at locations throughout the State of Oregon.
- Must successfully complete criminal history checks in accordance with Oregon Administrative Rules.

- Post-Offer Check: The final candidate(s) will require to successfully clear a third-party background check and oral and written bilingual proficiency tests (if applicable) as pre-employment conditions and Oregon State comprehensive background check as a condition of continuous employment.
- Start Date: A start date will be determined after all pre-employment condition(s) have been met.
