Job Description

Pay: $19/hour

Work Schedule: Monday-Friday 7:30am-4:00pm

JOB SUMMARY:

The Supervisor II, Lead Case Management, will be responsible for providing directions and guidance to Case Managers. This individual will provide support to ensure the needs of the participants at the affiliated Goodwill of Colorado Day Program sites are being met to maximize their fullest potential in their program environment.

ESSENTIAL FUNCTIONS:

Case Management:



  • The Lead Case Management Supervisor II will provide direction, supervision, and case management for all assigned participants to include: completing intakes, completing an initial plan and assessment, developing and implementing an individual program plan and document progress. The incumbent will function as part of the interdisciplinary team and develop the individual rehabilitation program. The Lead Case Management Supervisor II will encourage independence by assisting participants in problem solving by using accepted and innovative rehabilitation techniques and maintaining a proactive environment. The incumbent will also maintain good working knowledge of Goodwill's policies and procedures as well as state regulations and rules.


Staff Support and Development:



  • The Lead Case Management Supervisor II is responsible for all personnel issues for direct reports, including monitoring performance and completing performance evaluations. As a Team Lead, the incumbent will supervise direct reports and ensure all staff are present for work and held accountable for their job duties. The Lead Case Management Supervisor II will monitor the work of each staff member for accuracy and appropriateness while ensuring that all case management paperwork for CARF and funding agencies are turned in on appropriate due dates. The Case Management Supervisor II will also assist with direct reports with problems and conflicts concerning funding agencies, parents, guardians, and consumers. The incumbent will cover for staff when absent.


Client/Program Tracking and Reporting:



  • The Lead Case Management Supervisor II will provide oversight and maintain cases management required paperwork to include but not limited: possible mandatory reports to state, the local police department or Adult Protective Services (APS), if necessary.


Relationship Management:



  • The Lead Case Management Supervisor II will communicate and coordinate with other agencies involved and support personnel. In addition, the Lead Case Management Supervisor II will demonstrate good customer service with all clients. The Lead Case Management Supervisor II will deal effectively and calmly with non-routine situations following accepted guidelines and will perform other duties as required by the Coordinator. The incumbent will attend all meetings and relay pertinent information to the rest of the team. In addition, the Lead Case Management Supervisor II will also attend training courses and sign staff up for necessary trainings.


QUALIFICATIONS:

Education:



  • A Bachelor's Degree in the Human Services field is preferred.


Experience:



  • One (1) year of experience in managerial and/or administrative positions mandatory if degree preference is not met.
  • Prior experience working with individuals with intellectual and developmental disabilities is preferred.


Other:



  • The incumbent should have excellent written and verbal communication skills, write clear, concise, and accurate reports and plans, and submit them in a timely fashion.
  • Must demonstrate the ability to interact with various types of internal and external customers, relate effectively to disabled persons, their parents and guardians and be empathetic toward them.
  • Must have a working knowledge of community resources, demonstrate initiative, and possess excellent time management skills.
  • Must have the ability to work independently, cooperatively, and to exercise effective planning.
  • Must also conduct self in a professional manner and maintain a professional appearance that enhances the firm's reputation.
  • Must have experience speaking effectively to various small groups in a presenting/informative role.
  • Must be able to train and be creative in motivating participants toward individual success.
  • The incumbent will be required and must have the ability to drive for company business*.


*Per auto vehicle insurance carrier requirements: For applicants/employees to qualify for inclusion on the Goodwill driver insurance schedule, for purpose of driving Goodwill fleet vehicles, or for purpose of receiving company mileage reimbursement they must be at least:



  • 19 years of age (not engaged in passenger transportation),
  • 25 - 70 years of age for all passenger transportation services,
  • 21 years of age for CDL.


Applicants/employee will be required to undergo a Motor Vehicle Record (MVR) check and background check; applicants/employees must disclose all moving traffic violations or vehicle crashes (within the last five (5) years)(Note: not all violations are a disqualification for employment and each case will be reviewed by the insurance carrier); must have valid Colorado State driver license; must also be able to operate company vehicle, and adhere to all Federal, State, and local laws governing vehicle operation.

Applicants/employees using their personal vehicle for company business (receiving company mileage reimbursement), must have valid vehicle insurance, valid driver license, and vehicle must be in roadworthy condition.
