Position Status: Full Time - Day Shift Position Location: This is a float position that will cover locations in the Hampton Roads area where CTC is out of the office for reasons such as PTO or call out. Overview Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.
