Overview Works collaboratively with physicians, staff and other health care professionals to provide comprehensive care management and care coordination across the perinatal period health for the highest risk patients in order to improve outcomes and lower the cost of care. Is an integral member of the health care team who works to ensure safety, best practice and high-quality standards of care are maintained. RN Care Coordinators create longitudinal, personalized care plans for patients/family/support system, collaborate with and coordinate the efforts of care across the continuum, and increasingly use data analytics to manage the health of populations to improve patient access to care and clinical outcomes.
