Description The Division of Pediatric Pulmonary Medicine and the UNC Children's Pulmonary Clinic supports internationally recognized clinical and research programs in pediatric lung health.Clinical faculty and staff of the Division of Pediatric Pulmonology combine cutting-edge research, diagnostics, and tested treatment strategies to provide exceptional care to benefit the lives of children with various lung diseases. The Pediatric Pulmonary RN Care Coordinator (RNCC) plays a vital role in supporting patient care and clinical operations within the Division of Pediatric Pulmonary Medicine and the UNC Children's Pulmonary Clinic. As a consistent point of contact for patients and families, the RNCC provides comprehensive care coordination, including triaging clinical concerns, communicating laboratory results, processing medication refills, and managing documentation needs such as school forms and FMLA paperwork. The RNCC also facilitates prior authorizations, coordinates with home health and DME providers, and ensures seamless transitions of care between inpatient and outpatient settings. In collaboration with physicians, respiratory therapists, social workers, and other healthcare professionals, the RNCC develops individualized care plans and delivers patient and family education to support understanding of diagnoses, treatments, and symptom management. The RNCC participates in quality improvement initiatives, monitors patient outcomes, and contributes to process improvements that enhance clinic efficiency. This is a hybrid role. The expectation will be to work on site in either the Chapel Hill or Raleigh Clinic 2-3 days per week depending on coverage needs.
