Job Overview:

Social Worker assists patients and their families to obtain the maximum of available healthcare services through psychosocial assessment, counseling, education, and referral services. The Hemophilia Treatment Center Social Worker assists patients and their families to obtain the maximum of available health care services through psychosocial assessment, counseling, education and referral services. Responsible for the delivery of social work services for pediatric and adult patients and their families diagnosed with coagulation disorders.

Qualifications:

Required:

  • Master s degree in social work MSW from an accredited program preferred.
  • Valid state licensure LCSW or equivalent
  • Minimum of 2 years of experience in pediatric social work or a related field
  • Strong understanding of child development and psychosocial factors affecting pediatric patients
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work effectively in a fast-paced, multidisciplinary environment.

Essential Functions:

  • We are seeking a dedicated and compassionate experienced social worker to join our pediatric specialty outpatient department,
  • The ideal candidate will have a strong background in pediatric social work, excellent communication skills, and the ability to work collaboratively within a multidisciplinary team to provide comprehensive care to children and their families
  • Conduct thorough psychosocial assessments of pediatric patients and their families
  • Identify social, emotional, and environmental factors affecting the patients' health and well-being
  • Provide individual, family, and group counseling to address emotional and behavioral challenges
  • Offer crisis intervention and support to families in distress
  • Collaborate with healthcare providers to develop and implement individualized care plans
  • Facilitate referrals to appropriate community resources and services, including mental health services, financial assistance, and educational support
  • Advocate for the needs and rights of pediatric patients and their families within the healthcare system
  • Assist families in navigating insurance and healthcare-related challenges
  • Provide education to families regarding coping strategies and resources available to them
  • Participate in training and development opportunities to stay current with best practices in pediatric social work
  • Maintain accurate and timely documentation of patient interactions, assessments, and care plans in compliance with legal and ethical standards
  • Work closely with physicians, nurses, therapists, and other healthcare professionals to ensure a holistic approach to patient care
  • Participate in departmental meetings and contribute to quality improvement initiatives
