Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help "homebound" patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies.

Education
High School Diploma or Equivalent required and Bachelor's Degree Related Field of Study preferred

Experience
scheduling experience in administrative medical office setting 3-5 years required

In this role, you'll work collaboratively, managing referral workflows, communicating with healthcare providers, patients, and internal teams, and maintaining detailed records that ensure seamless patient transitions. You'll be supported by an experienced team of nurses, therapists, and care coordinators-all focused on delivering the highest standard of care.
