POSITION SUMMARY: The Family Partner serves as a key member of the Integrated Behavioral Health (IBH) team at GRMDC, supporting patients of all ages with a particular focus on pediatric care. This role centers on building trusting relationships with families to provide peer support, advocacy, and assistance navigating behavioral health services and community resources. Working closely with primary care and behavioral health clinicians, schools, and community agencies, the Family Partner helps reduce barriers to care, promote emotional well-being, and support child development. While not providing clinical treatment, they empower families to engage in care and connect to appropriate services, with a strong emphasis on cultural competence and collaboration across systems. Bilingual language capacity in Albanian, Spanish or Haitian Creole. Work towards attaining one of the CHW certification or CFP Certification within 6 months of hire. Compensation Range: $23.80- $34.38This range offers an estimate based on the minimum job qualifications.
