Unit Description: Registered Nurse (RN) or Licensed Practical Nurse (LPN) - Night Shift Post Acute Care Unit Helen Porter Rehabilitation & Nursing \| Middlebury, Vermont Full-Time \| 36 Hours per Week \| 6:45 PM - 7:15 AM \| Rotating Weekends Join a Team That Delivers Exceptional Post-Acute Care Helen Porter Rehabilitation & Nursing, part of the University of Vermont Health Network and Porter Medical Center, is seeking an experienced and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Post Acute Care Unit on the night shift. Our post-acute unit provides skilled nursing and rehabilitation services to patients recovering from surgery, illness, injury, and hospitalization, helping them regain strength and independence before returning home. As a member of our nursing team, you'll play a vital role in delivering high-quality clinical care while supporting patients through an important stage of their recovery journey. Located in beautiful Middlebury, Vermont, Helen Porter Rehabilitation & Nursing is known for providing exceptional skilled nursing, rehabilitation, and long-term care services in a welcoming, resident-centered environment. Schedule Full-Time 36 hours per week Night Shift: 6:45 PM - 7:15 AM Rotating weekends Three 12-hour shifts per week What You'll Do As a Night Shift RN or LPN on our Post Acute Care Unit, you will: Provide skilled nursing care to post-acute and short-term rehabilitation patients Assess, monitor, and document patient conditions and response to treatments Administer medications, IV therapies, and treatments as ordered Collaborate with physicians, nurse practitioners, therapists, and interdisciplinary team members Support patient recovery goals and discharge planning Respond to changes in patient condition and communicate effectively with providers Promote patient safety, comfort, and quality outcomes Maintain accurate electronic medical records and nursing documentation Qualifications Registered Nurse (RN) Current Vermont RN license or eligibility for Vermont licensure Associate's or Bachelor's degree in Nursing Licensed Practical Nurse (LPN) Current Vermont LPN license or eligibility for Vermont licensure Preferred Qualifications Experience in skilled nursing, rehabilitation, post-acute care, medical-surgical nursing, acute care, long-term care, or healthcare settings Strong assessment, communication, and critical thinking skills Ability to work collaboratively as part of a multidisciplinary team Commitment to compassionate, patient-centered care Our nurses enjoy: Competitive compensation Night and weekend differential opportunities Comprehensive benefits package Medical, dental, and vision insurance Retirement savings plans Paid time off Tuition assistance and professional development opportunities Career growth within the University of Vermont Health Network Supportive leadership and collaborative work environment Meaningful relationships with patients and colleagues About Helen Porter Rehabilitation & Nursing Helen Porter Rehabilitation & Nursing provides skilled nursing, rehabilitation, and specialized care services for residents and patients throughout Addison County and beyond.
