Job Announcement

Join a dedicated team where your clinical judgment, mentorship, and commitment to person-centered planning help individuals thrive in their communities! This position supervises a team of Support Coordinators and provides clinical direction to them in areas including clinical assessment of client needs (including treatment modalities, intervention techniques, behavior management, independent living skills, supportive counseling, family education, crisis intervention, housing, communication, recreation, financial, medical, transportation, vocational, legal, education, etc.); Interdisciplinary Team Leadership; formulating, writing and developing Person Centered Plans; continuous individual progress review and evaluation, use of assessment tools; referral methods; case consultation, documentation, and special projects. Assists DDSII Support Coordinators in determining clinical interventions and in problem solving in order to enhance their knowledge, abilities, and clinical skills. Supervises Support Coordinators administrative duties. Provides training and education regarding inter and intra-agency services and clinical interventions. Provides oversight and review of submissions required to maintain services under the DD Waivers (Service Modifications, Renewals, etc.) including submissions in WaMS (Waiver Management System). Participates in record and utilization reviews. Assumes caseload duties in the absence of supervisees. Provides Crisis Intervention Assistance. Participates in Emergency 24-Hour on-call rotation for service provision. Oversees the use and maintenance of the electronic health record to record all aspects of Support Coordination performance as well as to document the delivery of services.

Illustrative Duties

For Positions in CSB Residential Services:
Ensures that facilities are well maintained and in compliance with governing standards.

For Positions in CSB Residential and Support Coordination:
Provides clinical consultation, supervision, evaluation and oversight to lower level specialists;
Makes clinical assignment of cases based on assessment of client needs, staff experience and caseload levels;
Consults with professionals on a variety of problems including delivery of needed services for clinically complex cases;
Ensures the proper development, implementation and review of individual service plans;
Communicates agency mission to subordinate staff and advises on its implementation;
Ensures compliance with Medicaid regulations;
Prepares and/or reviews administrative reports, forms, and files to ensure compliance with program licensure and certification requirements;
Provides liaison to community agencies to facilitate communication and service provision for consumers requiring multiple services;
Acts as an advocate for clients;
Represents the agency on regional, county, state and multi-agency committees;
Investigates, analyzes, and reviews issues affecting service delivery;
Ensures adherence to, and implementation of, federal, state, local, and agency regulations and licensure standards;
Represents service area director in management team and other meetings.
Maintains ongoing communication with families, landlords, county programs and agencies, and state agencies.


Required Knowledge Skills and Abilities

Extensive knowledge of principles, practices, methods, and techniques related to treatment of persons with intellectual disability or other developmental disabilities;
Knowledge of major methods of treatment of developmentally disabled individuals;
Knowledge of intake procedures, social history development and interviewing techniques;
Knowledge of community resources, including relevant public and private organizations;
Knowledge of psychiatric, psychological, and/or sociological terminology and concepts;
Knowledge of federal, state, and local laws and regulations, as well as issues relevant to human rights, mandated reporting, and skill in applying this knowledge to a variety of situations;
Ability to supervise and train service professionals;
Ability to conduct performance evaluations and implement personnel procedures;
Ability to successfully perform as a team member/leader;
Ability to recognize external behavioral symptoms of developmentally disabled individuals who may concurrently exhibit signs of mental illness or substance abuse;
Ability to establish rapport and maintain effective relationships with clients;
Ability to develop and maintain effective working relationships with subordinates, co-workers, public and private sector organizations, community groups and the general public;
Ability to recommend alternatives to allow for individual choice in services;
Ability to communicate effectively, both orally and in writing;
Ability to maintain records and prepare reports.


Employment Standards

MINIMUM QUALIFICATIONS:
Graduation from an accredited college or university with a bachelor's degree in psychology, social work, or a related field; plus five years of experience working with individuals with developmental disabilities;
or bachelor's degree and a valid Qualified Developmental Disabilities Professional (QDDP) certification plus five years of experience working with individuals with developmental disabilities.
Masters can be substituted for 1 year of experience; or graduation from an accredited college or university with a doctoral degree in psychology, social work, or a related field

CERTIFICATES AND LICENSES REQUIRED:

  • Valid motor vehicle driver's license with fewer than six demerit points (or equivalent in another state) maintained throughout employment with CSB.
  • CPR certification required within 3 months after appointment and maintained throughout CSB employment.
  • Qualified Developmental Disability Professional (QDDP) certification required within 1 month of hire.
  • Qualified Intellectual Disability Case Manager (QIDCM) certification required within 1 month of hire.


NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter. Eligibility to provide services that can be reimbursed by Medicaid in accordance with the Virginia Department of Medical Assistance Services' requirements.

This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.)

PREFERRED QUALIFICATIONS:
  • Bachelor or higher degree in a human service-related field.
  • Experience with service delivery under Medicaid and Medicaid Waivers.
  • Current certification as a Qualified Developmental Disabilities Professional or Qualified Intellectual Disabilities Case Manager (QIDCM) certification


PHYSICAL REQUIREMENTS:
Ability to drive to numerous locations in the community during the course of the day (include residential, vocational, and other facilities); use keyboard driven equipment; and communicate clearly with others orally and in writing. All duties performed with or without reasonable accommodations.
