Summary Job Description: Seeking RN Case Manager! PRN (work as needed), shifts and days vary Baptist Health Floyd, Care Coordination Southern Indiana/Louisville, Kentucky area This position leads the collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet patient' health needs through communication and use of available resources. The goal for this position is promotion of quality cost-effective outcomes including access to care and appropriate utilization of resources. A case manager is an expert and vital participant in the care coordination team who empowers people to understand and access quality, safe, and efficient health care services. Bachelor's degree preferred.
