The Johns Hopkins Hospital - Pediatric Nurse Case Manager - Main Hospital 40hrs per week, 8a-4:30p, Monday through Friday No weekends or holidays required No On Call Requirements Facilitating and coordinating care by providing evidence-based care based on the nursing process, policies, procedures, and protocols of the Johns Hopkins Health System. Providing direction to/management of other team members to promote quality, safety, and excellence in care. Working collaboratively with, and providing leadership for, a multidisciplinary health care team to support program initiatives that support quality, safety, and excellence in care. Participating in promoting improvement initiatives (clinical, financial, operational) that supports, protects and promotes area of specialty. Implementing initiatives in area of specialty as directed by nursing and medical leadership. Fostering a clinical environment of service excellence. Qualifications New need starting in January: The Johns Hopkins Hospital - Pediatric Nurse Case Manager - Main Hospital Required: ActiveRN License, (2) years RN experience, BLS,Currentin-patientCase Management experience; post-acute facility referral platforms such as NaviHealth or AllScripts. Preferred: EPIC, Case management certification Does not need Pediatrics experience but does need experience being a case manager and helping to facilitate discharge planning experience working at the bedside- preferably at least 5 years' experience. ICU experience preferred but not required Will Float to all Peds Units:Units to cover -JHCC (NICU, PICU/PCICU, Onc, 9N/S, 10N/S)- would not float to psych.
