The incumbent will develop, manage, and coordinate all aspects of the outpatient diabetes program and is responsible for outreach to physician practices as a referral source for the diabetes program.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education EXPERIENCE: 1. One (1) year experience in health and fitness PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. ASCM, Clinical Exercise Physiology Certified CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Identifies education needs of the community and those with diabetes. 2. Attends diabetes advisory committee meetings as assigned. 3. Performs patient assessment, delivers education and evaluates patient's obtainment of goals. 4. Assists in maintaining the American Diabetes Association certification as applicable. 5. Participates in community outreach and educational support. 6. Advocates on behalf of the patient and families. 7. Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes. 8. Participates in performance improvement activities to assure quality care. 9. Provides administrative support and coordinates the management and operations of the Diabetic Institute 10. Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program. 11. Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
