The Case Manager will perform screening interviews with clients wishing to gain entrance to the program. This position will maintain all client-related documentation, write case plans, meet weekly with each client and act as an advocate for the client with the Veterans Administration and outside agencies. The Case Manager will be responsible for presenting cases to the team for consultation. In addition, this position will be responsible for providing follow-up assistance as requested by the client for up to one year. The duties of this position also include information and referral activities as specified by the individual client's needs. Qualifications: Master's in Social Work (MSW), plus at least three years case management experience or equivalent work experience. Training and experience in homelessness, alcohol and other drug addiction, mental health, and experience providing intervention in those barrier areas. An ability to work with various socio-economic, ethnic, and racial populations a must; good written and oral communication skills; knowledge of social services network; commitment to non-violence in all interactions. Schedule: With the director's approval, establish a standard workweek taking into consideration the essential duties of this position, the mission of the department, and of the Salvation Army. Responsibility: Significant impact on program and costs. Work is done under general supervision in accordance with general procedures or policies but with latitude for the exercise of independent judgment. Decisions and actions are subject to prior approval in unusual cases. Supervisory Responsibilities: None. Work with and through Supervisor for all supervisory issues. Supervision is of a limited variety of activities of ordinary complexity. Organization: Must maintain an organized work area, files, etc. contributing to the efficiency and accuracy of work within the assigned workday and avoiding unnecessary overtime. Working Conditions: Must be able to comprehend assignments and/or ask for additional information or clarification until the assignment is understood. Position may require long periods of sitting at a desk using a personal computer and telephone. Able to walk, sit, stand, bend, on an intermittent or sometimes continuous basis. Must occasionally lift and move up to 25 pounds. While performing the duties of this job, the employee is regularly required to sit; use hands to finger, hand, or feel objects, tools, or controls; to talk or hear; reach with hands and arms. Specific vision abilities required by this job include close vision and distance vision. The applicant will work on multiple simultaneous projects in an environment characterized by interruptions and noise from ringing telephones, printers, copiers, and walk-in traffic, multiple deadlines, and competing priorities. Position involves frequent phone and in-person contact with people from diverse backgrounds who may be angry, hostile, confused, or frustrated people. Organizes projects, sets priorities, calculates, catalogues, inspects, analyzes, plans, comprehends, and cooperates with others, etc. Must be able to work with minimal supervision and be comfortable working with a high degree of independence. May be required to make independent decisions that may involve unique or unusual situations and/or complex issues and dealing with demanding requests and/or timelines. This position has access to confidential and private information. It is essential that confidentiality and privacy be observed. The Salvation Army has a dress code.
