POSITION SUMMARY:
Reporting to the Care Services Director, the Care Services Coordinatoris responsible forthe management of care services programs ensuring the uniform delivery of high quality, consistent services for individuals with ALS and their families within an assigned territory area. This positionfacilitateseducation, information, benefit, and clinical resource navigationassistancetopersonsdiagnosed with ALS and caregivers.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Utilize the Intake form to obtain all necessary client, medical, demographic information; document to support families in meeting goals.
  • Conduct care consultations and needs assessments for people living with ALS, caregivers, and families in a variety of settings including virtual visits, by phone/email, in clinic and support groups. Refers to Care Services Manager for complex case-expertise area.
  • Actively manage a portfolio of clients, assuring client and caregiver needs are met within the capabilities of The ALS Association.
  • Provide information on programs, services,resourcesand printed/electronic educational materials available topersonswith ALS and their families.
  • Coordinate referrals to ALS Association programs as well as healthcare providers and community agencies.
  • Facilitate resource support group meetings, engage participants, and schedule speakers asappropriate tomeet the needs of the group.
  • Document all pertinent engagements withpersonsliving with ALS, family, caregivers, or other healthcare professionals involved with the delivery of care and support in the database.
  • Ensure quarterly contact/engagement with those living with ALS and/or their caregivers.
  • Educate and provide those served on the available nationwide, state, and local resources throughutilizationof The ALS Association Resource & Referral Hub.
  • Serve as a member of the ALS Clinic multidisciplinary team and function as the liaison between the clinic and The ALS Association.
  • Participate in continuing education seminars, workshops, and conferences as available and necessary to gain knowledge on related topics.
  • Perform other relatedduties asassigned by management.

SUPERVISORY RESPONSIBILITIES

  • This job has no supervisory responsibilities.

QUALIFICATIONS:

  • Bachelor's Degree or related experience in social work, case management, health administration, allied and public health, nursing, ora related arearequired.
  • Minimum of 3 years related professional experience including direct care and/or case management experience.
  • Knowledge of Medicare, Medicaid, patient assessment skills, care planning, and case management preferred.
  • Knowledge of clinical and ethical regulations.
  • Must have and maintain a valid drivers' license.
  • Ability to work nights and/or some weekends (as required).
  • Experience with volunteer management preferred.
  • Strong computer skills and proficiency with Microsoft Office products
  • Good understanding of databases, such as Salesforce.
  • Commitment to organization, accuracy, attention to detail, highly efficient and conscientious about thorough
  • follow-up.
  • Skilled communicator, with superior interpersonal, verbal and written skills.
  • Able to handle confidential information and maintain high confidentiality.
  • Experience with home health and hospice is a plus

PAY TRANSPARENCY:
The ALS Association's pay range for this positionis$22.51 - $27.99 per hour.

The ALS Association has compensation ranges for various work locations throughout the United States, allowing us to compensate employees competitively and consistently in diverse geographic markets. The above-listed listed range shows prospective compensation for this role; the appropriate range will be established by what is eventually determined to be the candidate's primary work location. Various factors, including the complexity and significance of the role, job duties/requirements, relevant experience and abilities, and primary work location, determine individual compensation. Compensation ranges are evaluated and typically adjusted on a regular basis. Offers are made within the compensation range that is in effect at the time of the job offer.

The Association may, at its discretion, offer merit-based increases tied to individual performance. The benefits listed may differ depending on employment status with the Association. Healthcare benefits, a 401(k) plan with employer match, short-term and long-term disability coverage, basic life insurance, well-being benefits, paid time off and several paid holidays are available to Association employees, among other benefits.
