In collaboration with other members of the nursing team, the CIC Clinical Coordinator directs, monitors, and evaluates daily nursing care in accordance with established policies/procedures. This leadership role serves as a resource person for VCU Health CIC command center and Hospital at Home staff and models a commitment to the organization's vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall success. Programs within the CIC that this role would support include Hospital at Home, Remote Patient Monitoring, Ask-A-Nurse, and Post-Acute Nurse Triage. Of note, the role requires travel between our Northside location and the VCU Medical Center campus. There could also be at times travel to a patient's home environment (within a 25-mile radius of the VCU Medical Center campus). Key functions include: *Collaborates with subject matter experts and CIC triad Leadership team (Medical Director, Nurse Manager, Sr. Director) to create an environment of teamwork that supports improved outcomes, service, and staff engagement. *Leads daily IDT and sets staffing daily assignments in accordance with patient care needs and established productivity guidelines. *Supports clinical problem solving and medical escalations, covers for staffing needs. *Assists with Hospital at Home and RPM program admission and discharge processes to ensure efficient throughput and high quality, patient-centered care. *Assures quality in partnership with the Hospital at Home's network of integrated services including but not limited to dietary, pharmacy, infusion, laboratory, imaging, therapy, transportation, durable medical equipment, and personal care services *Supports proper inventory control and assists with managing supplies, technology, and equipment. Engages in problem solving: phones, technology, RPM equipment, etc. *Supports a patient-first philosophy and engages in service recovery when necessary. *Supports the efforts of the CIC to improve engagement by operationalizing current nursing strategies using employee rounding between multiple campuses and other initiatives. *Participates in the ongoing assessment of the quality of patient care services provided in the CIC command center, in collaboration with other members of the management team. The Clinical Coordinator facilitates patient flow, coordinates activities and communication for the clinical area nursing unit during his/her assigned shift. This person serves as a resource and leader for patients, staff, health care providers, administration and other disciplines. The Clinical Coordinator ensures that patient care is delivered to all patients in an efficient, caring and courteous manner.
